The Casa Colina Azusa Center proudly serves the San Gabriel Valley community, offering local access to many of the same physical rehabilitation services found on Casa Colina’s Pomona campus.
The Casa Colina Azusa Center is a hospital-based facility that is accredited by the Joint Commission. The center treats everything from common sports injuries, work-related injuries, and orthopedic injuries to neurological conditions as well as hand disorders. Our team of specialists will work to achieve the best possible results for patients.
Outpatient services provided at our Azusa center offer an individualized rehabilitation approach for patients who have suffered a wide range of disabling acute or chronic diseases, disorders, injuries, or physical impairments. Admission to outpatient rehabilitation services programs is appropriate when there is a compromise in function with potential for improvement and identifiable functional goals. Frequency of services is determined by the individual therapist upon assessment and detailed in the plan of care.
