Collaboration is at the very core of everything we do. Our acute rehabilitation clinicians work together in patient care teams that are dedicated to the treatment of brain injuries, spinal cord injuries, stroke, orthopedics and other diagnoses. The latter often includes individuals who may have become quite weak from a long hospital stay, joint replacement surgery, or have experienced a serious illness.
At Casa Colina Hospital’s Acute Rehabilitation Wing, all care is overseen by board-certified physicians specializing in physical medicine and rehabilitation. Our care experience is based on a case management nursing model in which a dedicated nurse coordinates each patient's care plan, communicates with family members, arranges follow-up appointments, and works with outside resources.
During your stay, everyone involved in your care is kept informed. Our clinical team members constantly communicate with each other and meet once a week to discuss your progress. They share what they've discovered about you or your loved one's response to inpatient treatment. They make adjustments. They brainstorm for better solutions. They establish short- and long-term goals with you.
Led by specialized physicians, our rehabilitation teams include experienced physical and occupational therapists, speech-language pathologists, neuropsychologists, rehabilitation nurses, dietitians, and more, all working together to achieve remarkable results and get our patients back to the things they love.
