Collaboration is at the very core of everything we do. Our clinicians work together in patient care teams that are dedicated
to the treatment of brain injuries, spinal cord injuries, stroke, orthopedics and other diagnoses. The latter often includes
individuals who may have become quite weak from a long hospital stay or serious illness.
At Casa Colina Hospital, physician-directed care is based on a nurse/case management model in which a dedicated nurse
coordinates each patient's care plan, communicates with family members, arranges follow-up appointments and works with
outside resources.
During your stay, everyone involved in your care is kept informed. Our team members constantly communicate with each
other and meet once a week to discuss your progress. They share what they've discovered about you or your loved one's
response to treatment. They make adjustments. They brainstorm for better solutions. They establish short- and long-term
goals with you.
We've discovered that when you have physicians, therapists, nurses, dietitians, a neuropsychologist, a pharmacist, and a
case manager concentrating on you or your loved one at the same time, remarkable results can occur.
